All required forms can be submitted electronically through your MyEBR account at a computer or through the Campanion App on a mobile device
(refer to page 9).
The following forms can be submitted online and will automatically be received by the Camp office.
• Yearly Camper Application
• Health History Form & Permission to Treat (part of camper health forms)
• 7-year jacket order form (campers enrolled for 7th summer)
The additional forms listed below need to be printed, completed and then scanned and uploaded into your MyEBR account. Once they are uploaded, they will be reviewed by the Camp office.
• New Camper Profile (1st year campers only)
• Physicians Form (part of camper health forms)
• Insurance Form (IMPORTANT: please attach a clear copy of both sides of your insurance
and prescription cards to the form before uploading)
Important note: Bunk placement and departure information for ‘upper camp’ (available in June) will only be issued after all the forms listed above have been received by the camp office. Your child will not be allowed to attend camp without completed health forms.
• We accept soft trunks or duffels
• All bedding must be packed in trunks. Note: all mattress pads & egg crates
WILL BE PACKED and returned home at the end of the summer.
• Toiletries, shampoo, etc., should be stored in unbreakable containers and packed in plastic bags to protect against leaking. As a reminder we are peanut, tree nut, and sesame oil free so toiletries must also remain free from these ingredients.
• Straightening irons, curling irons and hot plates are NOT permitted at camp.
